Completion / Removal


Removal of a grade of INC or 4.0 is done by passing an examination or meeting all the requirements of the course, after which the student shall be given a final grade based on his/her overall performance.Removal of INC or 4.0 should be done within the prescribed time of one (1) year. After one year, an un-removed INC will remain INC, while an un-removed 4.0 will automatically become 5.00.

Note that the official end of each semester / midyear term is as follows:
• 1st Semester - the day before the 2nd Semester registration period
• 2nd Semester - the day of the University Commencement Exercises
• Midyear Term - the day before the 1st Semester registration period

  Qualification The student must be officially registered (either with enrolled courses or in residence) on the date of completion/examination.
Processing period Any date set by the concerned faculty member, provided the completion of requirements or removal exam is within the one-year completion/removal period of the incurred INC/4.0.
Requirements
  • Form 25 Permit for Completion / Removal Examination
  • *Removal examination fee of Php 20 if the examination will be held outside of the completion period indicated in the UPD calendar (Student requests Statement of Account at OCS and pays at UP Cash Office.)
  • Mode of transaction Face to face or online (email)
      Procedure
    1. The student downloads UP Form 25 Permit for Completion / Removal Examination from the Downloadable Forms page of the CMu website.
    2. The student fills out the Form 25 until the payment part for all copies in the Sheet – Student’s Copy, Instructor’s Copy, College Secretary’s Copy. (He/She writes “N/A” if payment is not applicable.)
    3. The student emails or brings the Form 25 to OCS for the college secretary’s signature.
    4. The OCS personnel verifies if the student is eligible to complete/remove.If the student is eligible and the date is qualified, the receiving personnel writes “Valid until ” on the Form. (If there is an issue with the student’s status or date of completion/removal, the receiving personnel instructs the student on his her/next steps.)
    5. The college secretary signs the Form 25.
    6. The OCS personnel returns the Form 25 to the student.
    7. The student submits requirements for completion or takes the removal exam..
    8. Student writes the date of completion / removal exam and the name of the instructor (or the faculty member receiving the requirements or administering the completion exam).
    9. The instructor signs the Form 25.
    10. Student leaves the Instructor's Copy of his/her Form 25 to the instructor and submits College Secretary’s Copy to OCS.
    11. The instructor checks the student’s submission or examination and computes the student’s final grade.
    12. The instructor enters the student’s final grade in CRS.
    13. The student monitors his/her completion/removal grade in CRS.
